Registering as a patient

ACCEPTABLE FORMS OF IDENTIFICATION

The policy of this practice is to accept on to our list any person who is eligible to register permanently according to GMS Schedule 2 Regulation 3 (2)

  1. A photo ID e.g. Driving Licence
  1. Proof of Permanent Residence in the UK – Utility Bill, Bank Statement, Letter from Home office
  1. Medical Card from Health Board
  1. Proof of permanent employment – letter from employer
  1. Students – Letter from educational institute
  1. Entry visa in passport if period covered is for 12 months or more
  1. Any other document which confirms residency in the UK for 12 months or more
  1. Registration forms and Patient Questionnaire needs to be handed in at Reception (Please bring your photo ID and proof of permanent residence in with you).

OVERSEAS PATIENTS

  1. If you are registering for the first time in the UK please note that we will need to see your passport.  This should be handed in along with your registration forms so that we may obtain a copy for our records.
  1. Any students should also provide course information, which includes a start and finish date.
  1. Overseas patients who are working in Scotland should provide evidence such as a Contract of Employment, wage slip or letter from their employer.  If self employed, please provide a letter from Accountant/Solicitor or company invoices.
  1. Visas/Permits – Overseas visitors may also have appropriate UKBA (UK Border Agency) documentation to show that they are entitled to work, study or get married in the UK.  Forms IS96 for asylum seekers, ARC or SAL.

When registering, please bring your NHS medical card (if you have one) to the practice and those of any other persons registering with you. You will be asked to complete a registration form for each person registering with the Practice. In addition, all patients will be asked to complete a new patient questionnaire, allowing us to provide medical care in the interim period, while your medical records are transferred from your old practice to this one.

Everyone will be offered a registration medical appointment but we will ask you to make one if you are on repeat medication or are moving from another practice in Perth.

If you move out of the practice area, you will need to register with a new practice as soon as you move. We can provide information about how to find a new practice.

Preference of practitioner

Patients are registered with the practice, not an individual GP. For administrative reasons your medical card will be issued in the name of one of the doctors; however, you can at any time express a preference for a particular doctor, for either all of your medical needs or on a case-by-case basis. This preference will be recorded in your medical notes and we will do our best to respect your choice. However, not all the doctors in the practice provide all services and specific doctors may not be immediately available.
